It’s not my intent to hijack this thread, but I was having the exact same problem after installing iTunes under WinXP.
Whenever I tried to mount an image, the machine just went out to lunch and didn’t do anything other than let me choose a drive letter to mount to. So I uninstalled through Add / Remove programs, then rebooted and re-installed – same nothingness. Searched and found this thread, and here’s what worked for me:
Uninstalled WinCDEmu through Add / Remove Programs.
Rebooted.
Ran the fix as specified above ( http://wincdemu.sysprogs.org/wcdcleanup.exe ).
Rebooted.
Reinstalled WinCDEmu.
Rebooted.
Successfully mounted an .iso image to drive letter V:

One note, though – at all levels of removal, “Select drive letter & mount” was always present in the Context Menu – just something you may want to add to the wcdcleanup routine.
EDIT: seems the “Select drive letter & mount” entry in the context menu may have been from the ‘Sonic RecordNow!’ software I had installed on my machine.
WinCDEmu is a great little program – very much appreciated!
