In case anyone is interested in knowing, one must run visual ddk as admin in windows7/vista in order to get the menus to appear. I believe this to be an issue related to uac and the initial install runnig as admin.

There may be a way to fix this by setting the permissions manually – but the error that appears in the visual studio log when run points to a permissions error while opening app_data.
